Warning: Only book through the official psa.gov.ph portal. Fake “PSA booking” pages on Facebook charge fees for slots — PSA never charges for the appointment itself, and no legitimate fixer can reserve a slot for you.

Note: if your request involves a Court Decree or Legal Instrument, the system requires you to book at the PSA East Avenue outlet in Quezon City — provincial outlets cannot process these.

Knowing the flow inside the outlet saves you time and stress. Here’s how a typical visit goes:

  1. Arrive 15–30 minutes early. Guards check your appointment slip and ID at the entrance before letting you in — this is where the “no appointment, no entry” rule is enforced.
  2. QR code scanning. Staff scan the QR code on your slip and cross-check it against the booking database. A screenshot on your phone works, but a printout is safer if your battery is low or the screen is cracked.
  3. Fill out the request form. You’ll state the document owner’s full name, date and place of birth (or marriage/death), and parents’ names. Bring these details written down, especially if you’re requesting for a family member.
  4. Pay at the cashier. ₱155 per copy for birth, marriage, and death certificates; ₱210 for CENOMAR. Bring small bills — cashiers often run short of change in the morning.

  • Book at slot-release time, not at midnight. Slots are released in batches during working hours, not continuously. Checking at 8:00 AM on a weekday beats refreshing at 11 PM.
  • Match your booking name to your strongest ID. If your passport says “Ma. Teresa” and your voter’s ID says “Maria Teresa,” book with the name on the ID you’ll actually bring. The counter check is exact-match.
  • Write the record details down before booking. Full name as registered, date of birth, place of birth (municipality and province), and both parents’ full maiden/legal names. Guessing at the counter causes “record not found” results even when the record exists.
  • Requesting for a parent or child? Bring proof of relationship. Your own birth certificate or the child’s, plus your ID. Requests for non-immediate family are heavily restricted — don’t waste a slot on one.

If you’re outside the Philippines, you don’t book a CRS outlet appointment — you have three realistic routes instead:

  • Order online with international delivery. PSA Serbilis (psaserbilis.com.ph) accepts orders from abroad and couriers SECPA copies internationally. This is the standard route for most OFWs. Delivery abroad takes longer and costs more than domestic delivery, so order well before your deadline.
  • Authorize a family member in the Philippines. An immediate family member can book their own appointment and request your document with an authorization letter, a copy of your ID, and their own valid ID.
